The Registration Form to attend Mapúa World 2013 in Honolulu, Hawaii, on March 14 to 17 is now available for download. To inform us of your intention to attend the convention, download and complete the Registration Form and send it to us either by snail mail (using the postal system) or email it (as a PDF File).If you choose to mail it to us, our mailing address is:

Attendance of Mapua World 2013 will only be confirmed upon our receipt of your Registration Form and the accompanying Registration Fee which is US $150.00 per person if paid on or before December 14, 2012. Otherwise, it will be US $170.00 per person if paid beyond this date. When we receive your Registration Form and Fee, your name will be included in the List of Attendees in the Mapua World 2013 Website. The Registration Fee includes your participation in the 'Meet & Greet Night', 'Gala Night', 'Seminar', and 'Picnic'. Dinner is included during the 'Meet & Greet Night' and the 'Gala Night' as well as lunch during the 'Picnic'. The hotel room reservation, tour & luau at the Polynesian Cultural Center, technical and other tours, transportation, golfing fees, and all other meals and activities are not included.
An accompanying spouse or guest who will be attending Mapua World 2013 will also be required to complete and submit their own Registration Form and Fee. It is preferable and highly requested that when sending your Registration Form by email, send it as a PDF file. This will allow us to print the form in its actual size. Avoid sending it as a JPEG or GIF file since these are only suitable for photographs and graphics.
When mailing your payment (Check, Bank Draft, or Money Order) for the Registration Fee, be sure to include a copy of the completed Registration Form.
